Your cat went to his wellness exam last year and everything was fine. Then one morning he's breathing strangely from across the room, mouth slightly open, sides moving more than they should. Or you wake up and he can't use his back legs and he's crying in a way you've never heard. These are the moments hypertrophic cardiomyopathy announces itself, and they're terrifying partly because the cat looked completely normal the day before.
HCM is the most common heart disease in cats, and it's a master of staying invisible. The walls of the heart thicken slowly over months or years, the cat compensates without giving any outward sign, and then something tips the system over. The good news is that quiet, screened HCM is a very different situation than HCM caught in crisis. Cats diagnosed early often live years on appropriate medication. Cats diagnosed in the emergency room have a much harder road. That gap is why we screen, especially in higher-risk breeds and middle-aged or older cats.

- Most common feline heart disease, and it usually develops silently over years.

What is HCM and what does it actually do to the heart?
In hypertrophic cardiomyopathy, the muscular walls of the heart (especially the left ventricle, the main pumping chamber) become abnormally thickened. This isn't the same as the normal muscle growth a cat gets from being active. It's pathologic thickening that reduces how much blood the chamber can hold between beats. The heart compensates at first by beating faster and harder, but those compensations create their own problems. Blood backs up. The left atrium (the chamber above the ventricle) stretches as it works harder. Clots become more likely. Fluid eventually leaks into or around the lungs.
The disease typically moves through a few stages:
- Subclinical phase. Wall thickening is present but the cat shows no clinical signs. This phase can last years.
- Mild functional changes. Subtle drops in stamina that owners often attribute to aging.
- Atrial enlargement. The left atrium stretches under the load. This is the stage where thromboembolism risk becomes meaningful.
- Congestive heart failure or thromboembolism. The clinical crisis that often brings a cat to attention for the first time.
HCM is managed, not cured. Where your cat falls in those stages at diagnosis is one of the biggest factors in prognosis. Cats found in the subclinical phase often live years with monitoring and appropriate medication. Cats first diagnosed during a crisis face a more uncertain future. This is why we screen.
Which cats are most at risk?
A few patterns tilt the odds:
- Age. Most cats with clinical HCM are middle-aged or older, though the wall thickening itself can start earlier.
- Sex. Male cats are disproportionately affected.
- Breed. Maine Coons and Ragdolls have identified genetic mutations linked to HCM, and breeds like British Shorthairs, Persians, and Sphynxes also show elevated rates.
- Genetics in mixed-breed cats. Most cats with HCM don't have an identifiable specific genetic cause, and the disease shows up across every population, including mixed-breed cats with no known predisposition.
Being in a higher-risk group isn't a reason to panic. It's a reason to screen earlier and more thoughtfully. The cats we catch during routine screening have far more treatment options than the cats whose HCM announces itself in the ER.
What are the signs of HCM in cats?
This is one of the more deceptive conditions in feline medicine. Cats hide illness exceptionally well, and they reduce activity gradually in ways that look like normal aging from the outside. Many HCM cats look fine right up until they don't.
The subtle signs you might notice at home
The early signs are vague and easy to dismiss:
- Less playing, less jumping, sleeping more
- Slightly more breathing effort after mild exertion
- Hiding more than usual
- Reduced appetite
- Mild weight loss
You know your cat better than anyone. The cat who used to greet you at the door but now stays in the bedroom, or who used to land on the counter without thinking but now eyes it and decides against it, is worth mentioning to us even when the change feels too vague to count. We'd rather hear "she just doesn't seem like herself" and find nothing than hear it a year later when something has tipped.
A specific note on breathing: panting in cats is never normal. Unlike dogs, cats don't pant to cool off in routine situations. A panting cat is signaling significant respiratory effort, and that's a same-day call.
The emergency signs
Some signs mean now, not later:

Saddle thrombus and acute congestive heart failure are true emergencies, and these are not signs that wait until morning. How do we diagnose HCM?
No single test pins HCM down on its own. The workup for heart disease combines what we hear in the exam, a blood biomarker, and imaging.
Heart murmurs and what they actually mean
A heart murmur found at a wellness exam is often the first hint. But murmurs in cats are tricky, and a few things are worth knowing up front:
- No murmur doesn't mean no disease. Plenty of cats with significant HCM have no audible murmur at all.
- A murmur isn't proof of disease. Some cats with murmurs have completely normal hearts on imaging.
- A gallop rhythm is a bigger flag. An abnormal third heart sound is more concerning than a typical murmur, and we don't wait on a gallop.
- Loud doesn't mean severe. Murmur intensity doesn't always track with how much disease is there.
A murmur starts the conversation about further testing. It isn't an answer on its own.
NT-proBNP blood testing
NT-proBNP is a blood biomarker that goes up when the heart muscle is stressed. It's not diagnostic on its own, but it's a useful filter:
- Slips into wellness bloodwork without much added cost or hassle
- Flags cardiac stress before structural changes might be obvious
- Points us toward echocardiography when elevated
- Lowers (without completely ruling out) the likelihood of a cardiac cause when normal in a symptomatic cat
It's particularly valuable as a screen for cats in higher-risk groups, where echo for every cat every year would be a lot to ask. An elevated proBNP is what often tips the conversation from "let's keep an eye on it" to "let's image the heart."
Echocardiography: the test that actually answers the question
Echocardiography, or cardiac ultrasound, is the gold standard for diagnosing and staging HCM. It directly measures wall thickness, chamber dimensions, valve function, and how well the heart contracts and relaxes. For most cats, it's surprisingly easy. The cat lies on her side, we clip a small patch of hair over the chest, apply ultrasound gel, and run the probe across the chest from several angles. The whole thing usually takes 20 to 40 minutes. Most cats tolerate it without sedation, though some need a mild dose to stay still.
The results tell us whether HCM is present, how thick the walls are (mild, moderate, severe), how stretched the left atrium has become (which drives the clot risk conversation), how the heart is functioning overall, and whether there are early complications like fluid around the heart or a visible clot in the atrium. That's the information that drives the treatment plan. We provide echocardiography for cats right in our hospital, so referrals to specialty centers aren’t needed to get answers.
What complications can HCM cause?
The wall thickening is the underlying disease. The complications that develop from it are what cause most of the dramatic clinical signs and emergencies.
Congestive heart failure
As the left ventricle stiffens and the left atrium stretches, pressure backs up into the lungs and fluid leaks out. The signs of congestive heart failure usually come on fast:
- Resting respiratory rate climbing over 30 breaths per minute
- Effortful or open-mouth breathing
- Coughing (less common in cats than dogs, but possible)
- Fluid building up around the lungs (pleural effusion)
- Sudden, often dramatic decline from looking fine to looking very not fine
CHF is a true emergency. Cats in active heart failure need immediate diuretics, oxygen, and a calm, low-stress environment. The good news is that most cats can be pulled through the acute episode with prompt treatment, and ongoing medication then keeps the underlying condition managed. The hard part is recognizing the crisis fast enough.
Aortic thromboembolism (saddle thrombus)
A saddle thrombus is one of the most painful and frightening complications in feline medicine. A clot forms in the enlarged left atrium, breaks loose, and travels down the aorta until it gets stuck where the aorta branches into the arteries feeding the back legs. The classic picture is sudden and unmistakable: a cat who was fine an hour ago is now crying, dragging her back legs, paws cold and pale, no pulse where there should be one. Sometimes only one leg is affected.
Some cats will have warning signs before a full clot dislodges. They’ll have mini-clot “showers”, which may show up with unexplained behavior changes like hiding, vocalizing, disorientation, and gait changes or wobbliness. These events are absolute emergencies.
HCM is the most common underlying cause of saddle thrombus in cats. Treatment focuses on pain control, anticoagulation, and supportive care, and outcomes vary widely. Some cats recover function. Others don't. For HCM cats with enlarged atria, we often add an anticoagulant medication to reduce clot risk, though even with medication the risk doesn't go to zero.
Arrhythmias and high blood pressure
A thickened heart is more prone to abnormal rhythms, and cardiac arrhythmias can develop alongside the structural disease and contribute to symptoms. Older cats also commonly have systemic hypertension, often secondary to kidney disease or hyperthyroidism, and high blood pressure puts more load on a heart that's already working harder than it should. Blood pressure measurement and treatment are part of cardiac care for any senior cat with HCM.
How is HCM treated?
There's no cure. Treatment aims at slowing progression, managing complications, lowering clot risk, and keeping your cat comfortable. The heart disease medications we reach for most often:

Not every HCM cat needs every medication. We tailor the plan to the cat's stage, complications, and how they tolerate each drug, and the plan evolves as the disease does. A cat on a single beta blocker at diagnosis may end up on three or four medications years later. That's not failure, that's appropriate adjustment.

What about HCM in senior cats with other conditions?
Senior cats rarely have just one thing going on. Hyperthyroidism is the big one that interacts with HCM, and the interaction is worth understanding:
- Untreated hyperthyroidism makes the heart beat faster, causing damage over time
- Treating the hyperthyroidism can unmask or worsen the HCM as the heart returns to its true baseline
- Both conditions affect blood pressure and kidney function
- Medication choices for one can shift what's safe for the other

How do I monitor HCM at home?
The single most useful thing you can do at home is track your cat's resting respiratory rate. It sounds small. It isn't.
Tracking resting respiratory rate
Resting respiratory rate is one of the most clinically valuable numbers a cat owner can give us. Here's how:
Wait until your cat is sleeping or completely relaxed. Watch the chest or side rise and fall. One full breath is one inhale plus one exhale. Count for 60 seconds, or count for 15 seconds and multiply by four. Write it down. Repeat daily or a few times a week. There are phone apps that make this even easier.

A sustained climb from your cat's own baseline matters too, even if both numbers fall inside "normal." If she usually rests at 20 breaths a minute and is now consistently at 28, the trend is the story. Bring it up.
Recheck schedule
For cats with diagnosed HCM, how often we see you depends on where the disease is:
- Stable subclinical disease: every 6 to 12 months
- Treated mild to moderate disease: every 3 to 6 months
- Recently changed medication: 2 to 4 weeks after the change
- Advanced or unstable disease: as often as monthly
Recheck visits typically include a physical exam, weight check, bloodwork (especially important for cats on diuretics, where kidneys and electrolytes need watching), blood pressure measurement, sometimes a repeat echo on a schedule, and a look at your home monitoring numbers. For cats not yet diagnosed but in higher-risk groups, preventive testing for senior pets typically builds cardiac assessment into the rest of senior wellness.
Common questions about HCM in cats
My cat has a heart murmur. Does that mean HCM?
Not necessarily. A murmur is a sound; it can come from structural heart disease, a congenital defect, or sometimes from something benign with no clinical meaning. The follow-up (NT-proBNP, sometimes echocardiography) is what determines whether the murmur is telling us something important.
My cat seems totally fine. Why screen?
Because that's exactly when we want to find HCM. Cats with HCM frequently look completely normal until the disease is advanced. Screening catches the disease at a stage where treatment has the most impact. The cats we diagnose at routine wellness exams consistently do better than the cats we meet for the first time in an emergency.
How long can a cat live with HCM?
It varies enormously. Cats with well-managed early disease often live multiple years with good quality of life. Cats first diagnosed during a crisis face a much harder situation. The stage at diagnosis tends to matter more than the diagnosis itself.
What about cats with the genetic mutation?
For Maine Coons, Ragdolls, and certain other breeds, genetic testing can identify cats carrying mutations linked to HCM. Carrying the mutation doesn't guarantee the cat will develop clinical disease, but it does warrant more proactive screening earlier in life. Breeders use this information for pairing decisions to reduce mutation frequency in their lines.
Is the medication going to be hard to give every day?
Most HCM medications come as small pills, and many cats accept them in a pill pocket, a small piece of food, or with the help of a pill gun. A few medications can be compounded into flavored liquids or transdermal gels for cats who genuinely won't take pills. We'll work through what your cat will actually accept, because medication only helps if it's getting in.
What does this cost over time?
The honest answer is that managing HCM is a real financial commitment, but it's typically very manageable spread across the year. Most HCM medications are inexpensive generics. The bigger costs are the echocardiograms (initial diagnosis and periodic restaging) and the bloodwork and exams that go with rechecks. Pet insurance helps significantly if your cat was insured before the diagnosis. We'll always lay out the plan and the costs up front.
Living well with an HCM diagnosis
A diagnosis isn't an immediate crisis. Most cats with well-managed HCM go on to live comfortably for years, with most of their daily life looking the same as it always did. They still purr in your lap, still come for breakfast, still find sunbeams. They just need daily medications, a watchful eye on their breathing, and a few extra check-ins through the year.
